New Patients Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

If you wish to register with the practice it is necessary that you live in our practice area.  Please speak to reception about practice catchment area.

A patient does not need to be “ordinarily resident” in the country to be eligible for NHS primary medical care. You will be asked to bring a form of identification (passport or driving license/birth certificate in case of a child) and proof of address e.g., a utility bill.

When applying to become a patient there is no regulatory requirement to prove identity, address, immigration status or the provision of an NHS number in order to register. However, there are practical reasons why a practice might need to be assured that people are who they say they are, or to check where they live. Seeing some form of ID will help to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto a new practice.

You will also be asked to fill out a new patient registration form (GMS1) and a new patient health questionnaire.  If you have been previously registered the providing NHS number makes it quicker to bring your notes from your previous GP.

Any person who is over18 need to come in person to register.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Changes to Personal Details

It is very important that you keep the practice informed of any change in address / name or telephone number.  Proof of change may be requested by the practice. This is very important as it can delay medical treatment.
